Transfer Declaration Form (French) (10 KB, PDF)
You'll need the free Adobe Acrobat Reader plug-in in order to view these files.
To complete a transfer of shares, you must carry out the following four steps:
- The current registered owner (transferor) must sign the back of the share certificate exactly as shown on front.
- Fill in the new owner's name (transferee) and address.
- The signature on the certificate must be guaranteed by a bank, trust company or investment dealer (credit unions and Alberta Treasury Branches are not acceptable).
- The new owner must complete the enclosed form - "Transfer Declaration".
Once these steps are complete, mail both the share certificate and declaration form to:
Computershare Trust Company
100 University Avenue
Toronto, ON M5J 2Y1
Or: (see below for other locations across Canada).
It is recommended that you send these documents by registered mail as they are very valuable and costly for you to replace.
Note: A transfer of shares may trigger tax implications to attribute the capital gain or loss and future dividends back to the transferor. You may wish to check with Canada Customs and Revenue Agency or your tax advisor.
